Having surgery is an important decision for you and that is why we require you to complete a number of forms and ask that you read the brochures provided. Here is a guide to our forms and brochures.

The Admission Form contains personal and payment information.

The Consent Form relates to your medical diagnosis and details of the treatment which will be provided to you. You can fill in most of this information yourself but the rest must be completed in consultation with your specialist. We also ask you to sign your consent to the treatment which you will receive.

The Health Questionnaire provides us with necessary details about your medical history.

We recommend you read the anaesthetic information pamphlet which you will find inside the back cover of the Important Pre-admission Information booklet.
